The Dentist's Role in Southern Brazilian Teaching Hospitals: A Grounded Theory Study.

The aim of this study was to develop a theoretical model to describe the role that the dentist plays in Brazilian hospital settings. This qualitative study was based on the grounded theory research method. Participants were a total of 27 individuals in three groups: dentists and general practice and oral and maxillofacial surgery residents; faculty; and other health professionals who interacted with dental patients in the hospital setting during the 2012-13 year. Data were collected in three teaching hospitals in the state of Santa Catarina, Brazil, through scripted interviews using open-ended questions. The stages of collection, codification, ordination, and integration of data were guided by constant comparative techniques to formulate a theoretical model. The codes generated were organized into seven categories: identifying the hospital as a working and teaching environment; recalling the dentists' professional pathways; reflecting on the dentist's work in the hospital; considering the political and organizational dimension of the role of the dentist in the hospital; understanding patients' life and health contexts; education and professional practice; and moving on towards interdisciplinary practices in the hospital setting. Integrating the categories led to a theoretical model called "The Dentist's Role in the Hospital Setting: An Interdisciplinary Construction." This theoretical model provides a framework to understand how dentists perform in the hospital setting, with a focus on interdisciplinary practice, which in this study was shown to be incipient and heterogeneous.

Saúde bucal do recém-nascido na maternidade: expectativas de puérperas e profissionais da saúde / Oral Health of the Newborn in the Maternity Ward: Expectations of Puerperal Women and Health Professionals

Objetivo: Conhecer as expectativas das puérperas e profissionais de saúde para a atenção à saúde bucal do recém-nascido (RN), no âmbito da Maternidade. Método: Estudo qualitativo-exploratório, com 31 participantes, 16 puérperas e 15 profissionais da Equipe de Saúde, da Maternidade do Hospital Universitário da Universidade Federal de Santa Catarina (HU/UFSC). Os dados foram coletados por meio de entrevistas individuais e analisados pela técnica da análise temática de conteúdo. As categorias encontradas foram orientações e cuidados à saúde bucal do RN, expectativas das puérperas para a atenção à saúde bucal e expectativas da Equipe de Saúde para o trabalho do Cirurgião-dentista na Maternidade. Resultados: Para as puérperas, a atenção à saúde bucal de seus filhos, desde o nascimento, significa a oportunidade de obter orientações para promover cuidado com a boca do RN. Suas expectativas para a atenção à saúde bucal, iniciada na Maternidade, está na possibilidade da ausência de cárie dentária futuramente e, como consequência, a beleza dos dentes e melhor relacionamento dos filhos com o meio social. Os profissionais discorreram sobre a troca de conhecimento entre o Cirurgião-dentista e a equipe no atendimento integral do RN internado. Porém com a diversidade de atribuições que já assumem, ressaltaram a necessidade da inserção do Cirurgião-dentista na equipe para que essa atenção possa ser viabilizada no ambiente da Maternidade, bem como, da capacitação da Equipe de Saúde nesta temática. Conclusão: As expectativas relatadas, tanto pelas puérperas quanto pelos profissionais da Equipe de Saúde, relacionam-se à lacuna da atenção à saúde bucal na Maternidade. As puérperas manifestam interesse em obter conhecimento sobre cuidados à saúde bucal e os profissionais almejam um trabalho interdisciplinar, inserindo a saúde bucal na atenção integral prestada ao RN.

Objective: To know the expectations of puerperal women and health professionals about the attention to the oral health of newborns (NB) in the maternity ward.Method: This was a qualitative-exploratory study comprising 31 subjects, 16 puerperal women and 15 health professionals of the University Hospital Maternity Ward of Santa Catarina Federal University (HU/UFSC), Brazil. The data were collected from individual interviews and reviewed by thematic content analysis technique. The obtained categories were oral health instructions and care to NB, new mothers? expectations concerning oral health attention to their babies and Health Team?s expectations concerning the dentist?s work at the Maternity Ward.Results: For the puerperal women, starting attention to the oral health of their children at birth is the opportunity to receive guidance on how to provide oral health care to NB. The mothers? expectations on the attention to oral health started at the Maternity Ward resides on the possibility of their children not having caries disease in the future and, therefore, having good dental esthetics and a better social relationship. The health professionals reported on the exchange of knowledge and experiences between the dentist and the health team during treatment of the NB before discharge from the Maternity Ward. However, in view of their multiple assignments, they emphasized the need to include a dentist in the Maternity Ward Health Team as well as receiving training on subjects related to oral health.Conclusion: The expectations reported by the puerperal women and the Health Team refer to the lack of attention to the oral health of NB at the Maternity Ward. The puerperal women showed interest in receiving instructions on oral health care and the health professionals would like to have an interdisciplinary approach, including the oral health in the care to NB.

Odontologia e Saúde Suplementar: marco regulatório, políticas de promoção da saúde e qualidade da atenção / Dentistry and supplementary health: regulatory framework, health promotion policies and quality of care

A partir do marco regulatório e de um panorama da odontologia na saúde suplementar, este artigo discute as especificidades do setor odontológico no que se refere às políticas de promoção da saúde e qualidade da atenção à saúde propostas pela Agência Nacional de Saúde Suplementar (ANS). A atuação do Estado na saúde suplementar se dá a partir das leis 9.656/98, que define relações entre operadoras, produtos e seus beneficiários e 9.961/2000, que cria a ANS. Concomitantemente, houve elevado crescimento da odontologia no setor, por conta de mudanças no exercício liberal da profissão, trazendo a necessidade de conhecer a lógica de organização dos serviços quanto à assistência prestada e ao modelo de atenção praticado. A ANS desenvolve ações para estimular as operadoras na implantação de programas de promoção da saúde, na busca por um modelo de atenção integral. Ao mesmo tempo, promove a Política de Qualificação na saúde suplementar, com ênfase na dimensão assistencial, porém na odontologia o enfoque da avaliação ainda é o atendimento individual e fragmentado. Por fim, o grande desafio da odontologia passa pelo seu fortalecimento enquanto política de saúde pública, acessível a toda a população, e pela qualificação da atenção odontológica na saúde suplementar.

Based on the regulatory framework and an overview of dentistry in supplementary health, this paper discusses the specifics of the dental sector with respect to health promotion policies and quality of health care services proposed by the National Supplementary Health Agency (ANS). The State's activities in supplementary health are based on law 9.656/98, which defines the relations between operators, products and their beneficiaries, and law 9.961/2000, which created the ANS. Concomitantly there was a great increase in dentistry in the private health plan market, because of changes in the practices of the profession. This required the need to know the logic of the organization of the services regarding the assistance provided and the model of care. The ANS develops measures to encourage operators to implement health promotion programs, striving for an integral care model. At the same time, it promotes the qualification policy of supplementary health care, with emphasis on the scope of care, though in dentistry the focus of evaluation is still individual and fragmented care. Indeed, the great challenge of dentistry is making it a public health policy, accessible to all, and the qualification of dental care in supplementary health.

[The participation of the dental surgeon in the multidisciplinary health team for child care in the hospital context]. / A participação do cirurgião-dentista em equipe de saúde multidisciplinar na atenção à saúde da criança no contexto hospitalar.

This is research of a qualitative nature that sought to analyze the perceptions of both the health team and users of the Pediatric Unit of the University Hospital of Santa Catarina Federal University with respect to the participation of dental surgeons in the healthcare of hospitalized children. Data were collected through interviews based on a form with semi-structured questions and analyzed by the content analysis technique and the analysis-reflection-synthesis process. Eight professionals from the health team, as well as seven caregivers and five hospitalized children took part in the interviews. The dental care provided to these children was given by students of the Undergraduate and Graduate Program in Dentistry, via an extension project of a preventive educational nature. Results revealed broad acceptance and perception of the importance of the participation of dentists in the context of the Unit as team members implementing the concept of comprehensive healthcare, and as support professionals for the health team in the care, streamlining and optimization of interdisciplinary work. Participants perceive the effective participation of dentists in the healthcare of hospitalized children as being very important and even essential.

A participação do cirurgião-dentista em equipe de saúde multidisciplinar na atenção à saúde da criança no contexto hospitalar / The participation of the dental surgeon in the multidisciplinary health team for child care in the hospital context

Pesquisa de abordagem qualitativa que teve como objetivo analisar as percepções da equipe de saúde e de usuários da Unidade de Internação Pediátrica do Hospital Universitário da Universidade Federal de Santa Catarina quanto à participação do cirurgião-dentista na atenção à saúde da criança hospitalizada. Os dados foram coletados em entrevistas com base em formulário composto por questões semi-estruturadas e analisados pela técnica de análise de conteúdo e pelo processo de análise-reflexão-síntese. Participaram: oito profissionais da equipe de saúde, sete acompanhantes e cinco crianças internadas. A atenção odontológica prestada a essas crianças foi desenvolvida por alunos do Curso de Graduação e do Programa de Pós-Graduação em Odontologia, através de um projeto de extensão de caráter educativo preventivo. Os resultados evidenciam a ampla aceitação e a importância da participação do cirurgião-dentista no contexto da Unidade como: membro da equipe para concretização do conceito de saúde integral; profissional de apoio à equipe nos cuidados e na dinamização e otimização do trabalho interdisciplinar. Os participantes percebem como muito importante e até fundamental a participação efetiva do cirurgião-dentista na atenção à saúde da criança hospitalizada.

Formação em odontologia e interdisciplinaridade: o Pró-Saúde da UFSC / Interdisciplinarity and dentistry training: UFSC's Pro-Health program

Na última década, a formação de profissionais de saúde tem sido reformulada com políticas de educação e de saúde promovidas pelos Ministérios da Educação e Cultura e da Saúde. Este artigo apresenta a experiência do Curso de Graduação em Odontologia da Universidade Federal de Santa Catarina (UFSC) na implantação do Programa Nacional de Reorientação da Formação Profissional em Saúde ­ Pró-Saúde. Como processo indutor, o programa direcionou as mudanças com vistas à integração ensino-serviço e à utilização de novas metodologias de ensino-aprendizagem, enfatizando a atenção básica. Na área de Odontologia, tais movimentos mostram-se necessários tendo em vista que, historicamente, tanto o sistema formador como o mercado de trabalho foram regidos pela iniciativa privada. Na UFSC, as atividades foram estruturadas em conformidade com os três eixos de desenvolvimento do Programa e impulsionaram o movimento de reforma curricular. O Pró-Saúde foi fundamental para a indução desse processo de mudança em uma perspectiva interdisciplinar, trazendo contribuições em todos os eixos (AU).

In the last decade, the training of health professionals was restructured with education and health policies promoted in partnership by the Ministries of Education and Culture and Health. This article presents the experience of the Undergraduate Dentistry Course of the Federal University of Santa Catarina (UFSC) gained in implementing the National Redirection of Vocational Training in Health - Pro-Health. As an inductive process, it led to changes, aiming at integrating learning and health service and at using new methods of teaching and learning, emphasizing primary care. In the field of dentistry, these movements seemed necessary, considering that, historically, both the educational system and the labor market were governed by private initiative. In UFSC, the activities were structured in accordance with the three axes of development presented by the program, and drove the movement to reform the curriculum. Pro-Health was essential for inducing the process of change in an interdisciplinary perspective, offering contributions across all axes (AU).
